Japan Begins Talks on Free Childbirth

Japan has begun detailed discussions on a new system to make childbirth free of charge, with the government aiming to introduce nationwide coverage by June 2028 and the establishment of a uniform national price for deliveries emerging as the central issue.

The revised Health Insurance Act, enacted in May 2026, includes provisions to establish a new system under which childbirth expenses will be fully covered by public health insurance.

Under the proposed framework, the government will set a uniform nationwide price for normal deliveries, with the full amount paid through the public health insurance system. The measure is intended to eliminate the financial burden of childbirth for expectant mothers.

The system will also provide cash benefits to pregnant women, taking into account situations where out-of-pocket expenses may still arise, including cesarean sections and other medical procedures.

At the first meeting on October 1, officials began discussing the criteria that medical institutions must meet to participate in the new system. The government also plans to conduct surveys and hearings to assess the financial conditions of hospitals and other facilities providing maternity services.

The biggest challenge is determining the appropriate nationwide price for childbirth services. The amount will need to take into account the operating costs of maternity facilities while ensuring that women can give birth without facing substantial medical bills.

The Ministry of Health, Labor and Welfare is aiming to launch the new system by June 2028, with further discussions expected to focus on pricing, eligibility requirements and the financial sustainability of maternity care providers.
